This is a very simple PHP based email form. It allows customers to make a booking, selecting the date from a multiple choice box.
It also includes Captcha check to prevent against robots.
After submitting, the php form sends a neatly formatted emailer to the website owner, informing them of the booking. This kind of form could easily be modified to store the values in a database, allowing you to track all bookings from an online control panel, sort by date, sort by name, and much more! The possibilities are endless!
This is a script that we created in php, allowing users to submit 'facts' about the website, which were stored in the SQL database.
Another script was used on the homepage of the website, which would display a random fact from the database.
